  • Sample Insurance and Endorsement *A $1 Million General Occurrence and $2 Million Aggregate minimum Certificate of Liability Insurance and Additional Insured Specific Endorsement naming the certificate holder is required and must be submitted with application and payment. The following certificate holders are: City of Los Alamitos, and its officers, employees, agents and volunteers (3191 Katella Avenue, Los Alamitos, CA 90720) and any other associated entities specific to each event/program.
